The Preserve, a skilled nursing, assisted living and memory care community, has earned a 5-star rating by the Center for Medicare and Medicaid Services for the Skilled Nursing Campus. The rating, from a possible low of 1 to 5, is based on a combination of results and data from three important sources: health inspections, staffing and quality measures.
The Center for Medicare and Medicaid Services independently determines national ratings for nursing facilities, with Medicare and Medicaid reporting data. The Preserve has been rated “Much Above Average” compared to nursing facilities nationwide.

Opened in 2020, the Volunteers of America community is part of a national nonprofit organization serving many for over 125 years. The community offers a continuum of care with its intimate assisted living and memory care neighborhoods, short-term rehabilitation services and long-term care nursing community.
The community sits on Hope Preserve, a health care campus planned to include multiple facilities and organizations dedicated to caring for seniors and for people who are chronically ill.
